Due to a limited ability to reach patients outside the catchment area of each hospice, HAU has an outreach model. Outreaches are managed primarily by community volunteer workers who coordinate and organise these once a month clinics. A team from HAU will leave, often early to reach their destination, carrying an necessary medications. Upon reaching the outreach site the team will be met by a crowd of patients patiently waiting. Often outreaches are outside, team members will set up where they can find some shade and begin seeing patients. At these clinics patients or their carers receive clinical assessments, have prescriptions reviewed and filled, receive spiritual and psychosocial support and counselling, hear a health talk or testimonial from a patient, enjoy a hot nutritious meal and those in need receive food supplements or other welfare support. Over 200 patients are seen this way every month; these are patients that would not have access to these vital services any other way.
